If you decide to join the study you will be asked to:
Download a mobile (iOS or Android) app (free) and register an account: You need to have the study app on your phone in order to participate in this study. After completing the electronic consent form you will first complete an electronic registration process. The registration process can be done through the study app or the study website. Registration will include entering your name, email address and other general information about yourself. As part of this process you will also confirm your agreement to participate in the study.
Health Surveys: We will ask you to answer questions about yourself, your medical history, and your current health. You may choose to leave any questions that you do not wish to answer blank.
Tasks: You will be asked to rate your symptoms and feelings on 10 items, each scored between 1-5 using a simple and intuitive flower-like interface. We would like you to complete this at least 2 - 3 times each week, each lasting approximately 30 seconds. In addition, you will have the option to:
- Set up and record a medications or task regime
- Record diary notes
- Play a cognitive and motor testing games
- Connect to wearable and other devices to record daily steps, blood pressure, weight or blood glucose measures
